Introduction

Suprapubic tube placement is a minimally invasive procedure to provide an alternate route for urinary drainage by inserting a catheter directly into the bladder through the abdominal wall. Procedure

During SP tube placement:

  1. A cystoscope is inserted into the bladder to provide direct visualization and confirm anatomy.
  2. The bladder is filled with sterile saline for optimal visualization and positioning.
  3. The bladder dome is identified using external palpation, and a spinal needle is advanced into the bladder under direct cystoscopic guidance.
  4. Local anesthetic is infiltrated along the needle path.
  5. A small incision is made in the abdominal wall, and the Choi trocar with an obturator and tear-away plastic sheath is advanced through the incision and into the bladder.
  6. The obturator is removed, and a 16 French silicone catheter is passed through the sheath and into the bladder. Placement is confirmed via the cystoscope.
  7. The balloon is inflated with 6-8 cc of sterile water, and the catheter is gently pulled back to secure the balloon against the bladder wall.
  8. The sheath is removed, leaving the catheter in place.
  9. The incision is closed with vertical mattress sutures, leaving a mesentery to secure the catheter.

The entire procedure typically takes less than 10 minutes and is performed in a controlled setting.
