Comprehensive Overview

Fournier’s gangrene is a rare but life-threatening bacterial infection that affects the genital, perianal, and perineal regions. The infection is characterized by rapidly spreading tissue necrosis (death) and can result in severe pain, swelling, and foul-smelling discharge. Due to its aggressive nature, Fournier’s gangrene requires immediate medical attention, as the infection can quickly lead to systemic complications like septic shock and organ failure.

At Crossroads Urology, we offer emergency evaluation and treatment for Fournier’s gangrene, providing life-saving care and aggressive management to halt the progression of this serious condition.

Causes of Fournier's Gangrene

Fournier’s gangrene typically occurs when bacteria enter the body through a break in the skin or a localized infection. Common contributing factors include:

  • Bacterial infection: Bacteria, most commonly Escherichia coli, can cause Fournier’s gangrene when they enter the body through small skin wounds or existing infections.
  • Compromised immune system: Individuals with weakened immune systems, such as those with diabetes, HIV, or those undergoing chemotherapy, are at a higher risk of developing Fournier’s gangrene.
  • Diabetes medications: Some newer diabetes medications, particularly sodium-glucose cotransporter-2 (SGLT2) inhibitors, have been linked to an increased risk of Fournier’s gangrene. Medications in this class include Invokana, Farxiga, Jardiance, and others.

Symptoms of Fournier's Gangrene

The symptoms of Fournier’s gangrene develop quickly and worsen rapidly. Early signs and symptoms may include:

  • Severe pain and swelling in the genital or perineal area
  • Redness and warmth at the site of infection
  • Foul-smelling discharge from the affected skin
  • Fever, chills, and rapid heartbeat
  • Rapidly progressing skin changes, including darkening or bluish discoloration, and skin breakdown

Diagnosis and Treatment

Fournier’s gangrene is typically diagnosed through a physical examination and medical history, alongside laboratory tests such as blood work and cultures of the infected tissue. Early diagnosis and aggressive treatment are critical to survival.

Treatment options include:

  • Antibiotics: Broad-spectrum antibiotics are administered intravenously to combat the infection and target the bacteria responsible for the necrosis.
  • Surgical debridement: Emergency surgery to remove dead and infected tissue is often necessary to stop the infection from spreading to other areas.
  • Hyperbaric oxygen therapy: In some cases, hyperbaric oxygen therapy may be used as an adjunctive treatment to promote healing and reduce the risk of complications.
  • Supportive care: Intravenous fluids, pain management, and wound care are essential components of the treatment plan.
